ECW Report Submission Form (ver 3)

Important Note

ECW direct grantees should use this form to confirm that their annual or final report templates in the ECW database are completed and ready for review by ECW Secretariat. ECW will consider this form to be the proof of "submission" of a specific required report.

Please be informed that this form should be submitted only after all the report templates (Narrative, Results, and Finance) are completed. ECW does not accept separate submissions of each report template. However, if the narrative and results templates are submitted by the "Lead" organization, an individual submission of the finance template is accepted.

General Information

What is the investment window of your grant?*

By using the drop-down menu, please select the Grant Reference Number (GRN) of the grant for which you are reporting. Please read carefully and follow the instruction below to correctly identify GRN. (For more details about GRN, please see here.)

Please select the investment window as you did above in the first box
Select the country of implementation in the second box (if AF, please select “Global”)
The name of your organization in the third box
The reference number of the grant that you are reporting in the fourth box

Please note that the default is set as AF, Global and AKF respectively in each box as an example; hence, please change the pre-selected information in each box to select your grant reference number from the drop-down menu by pressing the down-arrow on the very right of each box.

(A)	ANNUAL REPORT - We are reporting activities that were implemented from the grant start date until Dec 31, 2023.
(B)	FINAL REPORT - Our grant ended during the calendar year 2023 and we are submitting a report that covers the entire grant period.

Individual and Joint Reporting Aspects

Please read carefully and choose one of the three paragraphs below that best describes your organization.

(A)	We are submitting the report as an individual solo grantee. No other organisations were approved with our grant at the same time. We are submitting all three templates (narrative, results and finance) of the report. NB: If you are an AF grantee, you should be submitting only two templates (narrative and results).
(B)	We are submitting the report as an individual non-lead grantee. We are part of a group of organizations whose grants were approved at the same time for the same crisis in a given country. One of the organisations whose grants were approved with our grant is submitting the joint narrative and/or results templates on our behalf. Therefore, we are submitting only the finance template.
(C)	We are submitting the report as the lead grantee of the organizations whose grants were approved at the same time for the same crisis in a given country. We are submitting the joint narrative and/or results templates on behalf of other organisations as well as the finance template for our organization.
